OneCare Vermont Opt-Out
Why is my health care information being shared?
Blue Cross and Blue Shield of Vermont currently partners with OneCare Vermont, which is a statewide Accountable Care Organization (ACO). An ACO is a group of doctors, hospitals, and other health care providers who work together to provide high-quality care and reduce costs.
What is the value of OneCare Vermont?
OneCare does not change your health insurance coverage in any way. It just makes it easier for your doctors to work together and share information. You may still see any doctor, hospital, or other health care provider you choose, as allowed by your health plan. OneCare will simply help your providers work together to better coordinate your overall care.
How do I opt-out so my healthcare information is not shared with OneCare?
It is important to note that some of your health care information will be shared with OneCare in order to measure and improve the quality of your care. OneCare will protect your medical information, ensuring its safety.
If you do not want your health care information shared with OneCare, you may decline by opting-out electronically below.
ACO Opt-Out Form
You can opt-out electronically using our ACO Opt-Out Form. You will need your ID card to complete the form.
